IDEAL framework for surgical innovation 3: randomised controlled trials in the assessment stage and evaluations in the long term study stage

چکیده
The complexity of surgical procedures oft en poses challenges for conducting a rigorous and comprehensive evaluation. This paper considers the fi nal two IDEAL stages of surgical innovation. Surgical randomised controlled trials are oft en challenging to undertake and require careful consideration of the intervention defi nition, who should deliver it, and the impact of surgeon and patient preferences. In the long term study stage, better monitoring of surgical procedures is needed, along with improved surveillance of devices. Introduction The IDEAL framework describes the stages through which interventional therapy innovation normally passes: idea, development, exploration, assessment, and long term follow-up (also known as stages 1, 2a, 2b, 3, and 4, respectively). This paper focuses on the stages of assessment (specifi cally in relation to randomised trials) and long term follow-up. By the assessment stage, a new intervention will have shown early promise and be used increasingly by the surgical community; however, the intervention's relative benefi t compared with alternative approaches will be uncertain. At the long term follow-up stage, a surgical intervention will need further assessment owing to technical refi nements or to related devices or procedures being brought onto the market. Surgical procedures are conducted with an almost infinite set of subtle variations: surgeon training, team exper

Tailoring study design to each stage of surgical innovation: the ideal recommendations
The pathway of surgical innovation is complex. Inherent ethical and practical characteristics make scientific evaluation of new techniques or devices by a definitive randomized controlled trial (RCT) challenging. The IDEAL Collaboration (http://www.ideal-collaboration. net) Framework for evaluating surgical innovation describes a five stage process Idea, Development, Exploration, Assessment and...
متن کاملIDEAL framework for surgical innovation 2: observational studies in the exploration and assessment stages
The IDEAL framework describes the stages of evaluation for surgical innovations. This paper considers the role of observational studies in the exploration and assessment stages. At the exploration stage, the surgical intervention is usually more widely used, and observational studies should collect prospective data from multiple surgeons, deal with factors such as case mix and learning, and pre...

Background The evaluation of innovation in surgery is a complex process challenged by evolution of technique, operator learning curves, inconsistent procedural quality, and strong treatment preferences among patients and clinicians [1]. Given these challenges, the development of early-stage novel surgical techniques has been criticized for poor-quality study methodology and data reporting [2, 3...
متن کاملDemonstration of the IDEAL recommendations for evaluating and reporting surgical innovation in minimally invasive oesophagectomy.
BACKGROUND The Idea, Development, Evaluation, Assessment and Long term study (IDEAL) framework makes recommendations for evaluating and reporting surgical innovation and adoption, but remains untested. METHODS A prospective database was created for the introduction of minimally invasive techniques for oesophagectomy. IDEAL stages of development and evaluation were examined retrospectively in ...
متن کاملProtocol for the systematic review of the reporting of transoral robotic surgery
INTRODUCTION Transoral robotic surgery (TORS) has been adopted in some parts of the world as an innovative approach to the resection of oropharyngeal tumours. The development, details and outcomes of early-to-later phase evaluation of this technique and the quality of evidence to support its adoption into practice have hitherto not been summarised. The aim of this review is to identify and summ...
